### **Introduction**

The mental health and physical health of patients are deeply connected. At Rxbuild Mental Health & Wellbeing Foundation, we’re working to create smoother pathways for healthcare providers like you to connect patients to mental health support, helping them recover both physically and mentally.

We invite you to join our *Wellbeing Pathway Referral Network*. This will allow you to refer patients directly to the mental health services they need—quickly, easily, and effectively.

### **About Us**

We specialise in bridging the gap between physical and mental health, providing a range of services for patients who need help with mental health issues. Our referral network helps healthcare professionals—whether GPs, physiotherapists, or NHS providers—quickly connect patients to the right mental health care.

### **What We Offer**

As a Wellbeing Pathway Partner, your team will be able to refer patients to:

- **Mental health support**
- **Mental health physical pathways**
- **Wellbeing programs** (stress management, resilience, etc.)
